Readmissions - Data Analysis and Notification Tools To Minimize Medicare Penalties

The Centers for Medicare and Medicaid Services have implemented new rules that will penalize health care facilities for patients who have readmissions within 30 days. The newest service in OA's SMR suite is our Readmission Management module. Please contact us for a discussion and demo of this module.

This module consists of a web based application with a configurable rule engine and an iPad companion application for point of care data entry. You can also integrate our solution with your existing EHR system. Objective Arts has a long history in integrating health care systems to maximize communication and minimize data entry. This application approaches readmission from the perspective that a small amount of data can go a long way towards:

  • Identifying high risk admissions
  • Generating advice and protocols from predefined rules aimed at reducing readmission risk
  • Providing data to further identify strategies for controlling readmissions

Our solution does not impose a particular strategic model. There is no preexisting knowledge or treatment protocols in the system. Rather SMR lets you define the rules about how particular patient types should be approached, who needs to be notified of events relating to those patients, and what data is critical to measure on admission.

Manage the readmission challenge with:

  • Powerful tools for rule driven notification
  • Rule driven advice
  • Checklist generation based on rules that you define
  • Instant solution deployment with only a few weeks required for rule definition
  • Little to no training or organizational disruption. Most organizations can be up and running in a matter of days
  • No major system implementation. Just configuration and then go live
  • Little, if any, new data collection depending on your needs.

Readmission Rates will be a huge issue for hospitals beginning in 2012. Any readmission in less than 30 days will result in a financial penalty to the hospital. Speed to implementation is perhaps our most important metric. Every implementation is different, but our goal is to build a customized implementation of the SMR platform, with minimal work for the user overnight.
